Compassion & Choices
Compassion & Choices works nationwide to educate and advocate for expanded choices, ready access to them and improved medical practices that puts patients first and values quality of life in treatment plans for terminal illness. Additionally Compassion & Choices has several toolkits to assist individuals and their caregivers in considering care options.   

Five Wishes
Five Wishes is a complete approach to discussing and documenting your care and comfort choices. It’s about connecting families, communicating with healthcare providers, and showing your community what it means to care for one another.

Hospice Foundation of America
HFA’s mission is to provide information on issues related to advance care planning, hospice and palliative care, caregiving, and grief. Their resources for the public are to assist individuals coping with caregiving, terminal illness, and grief. 

Hospice Patients Alliance (HPA)

A private non-profit charitable corporation dedicated to serving the general public, hospice patients, their families/caregivers, the bereaved and hospice staff. HPA promotes quality hospice services whether a patient is enrolled in a licensed hospice, residing at home, or in a facility. 

National Hospice and Palliative Care Organization

The National Hospice and Palliative Care Organization (NHPCO) is the largest nonprofit membership organization representing hospice and palliative care programs and professionals in the United States.
